This will really save some time for everyone:
I just purchased two x3650 M3's configured with a QLogic iSCSI adapter model QLE406x and had an absolute nightmare of a time getting ESX to boot when installed:
Specific part num's:
42C1770 iSCSI QLogic card
ServeRAID 46M0831
x3650 M2 7945H2U
OS: ESX 4.1
Troubleshooting steps:
-changed slots
-updated uEFI firmware, ServeRAID firmware LSI, iSCSI firmware 1.14
-changed to a different card same model QLE406x
-Tried to boot to two diff ESX versions 4.0 and 4.1 along with Windows OS and got errors basically booting to Grub> prompt and getting Error 28: Selected item will not fit into memory
-Changed every FASTUtil setting on the QLogic card you can think of and IBM BIOS setting
Spoke with VMWare and IBM and finally got both on the phone and the specific IBM resolution from their knowledge base which is very difficult to locate is as follows:

VMWare support is working on KB, but this will save you two weeks, at least for me of troubleshooting.
